Before organising your fundraising event or challenge, please complete the online form below or download Lifeline's Community Fundraising Proposal and Agreement Form and return it to the Lifeline National Fundraising Team via email fundraising@lifeline.org.au or fax (02) 6215 9401.

By completeing this form you will be officially registering your event with Lifeline who can then provide you with information and tools to assist you with your event or challenge.
